Cleanings, X-Rays and Exams: The Cash Price
A routine dental visit is three separate services billed as one appointment. Knowing which is which makes the bill legible and makes it possible to ask for less when you need to.

Regular cleaning vs deep cleaning
These are different procedures with very different prices, and the upgrade is where most dental billing disputes start.
A prophylaxis is preventive maintenance for healthy gums. Scaling and root planing — the deep cleaning — is a treatment for periodontal disease: cleaning below the gumline, usually done in quadrants over multiple visits, frequently with anesthetic, at $200 to $400 per quadrant.
If you are told you need a deep cleaning, ask to see the numbers. Periodontal diagnosis is based on measured pocket depths recorded around each tooth, plus bone loss visible on X-ray. A practice diagnosing periodontitis should be able to show you those measurements. Asking is reasonable, common, and the fastest way to tell a clinical recommendation from an upsell.
Paying cash sensibly
- Ask for the self-pay price when you book. Many practices have one below their insurance fee schedule and do not advertise it.
- Ask whether X-rays are necessary this visit — frequency should follow risk, not ritual.
- Ask about a practice membership plan. Many offices now sell their own: two cleanings, exams and X-rays for a flat annual fee, often a few hundred dollars.
- Bring previous X-rays when changing dentists. They are yours, and it avoids paying twice.

How often do I need X-rays?
Guidance is risk-based rather than fixed. Low-risk adults need bitewings less often than high-risk ones. It is reasonable to ask why a given set is being taken.
Is a deep cleaning ever a scam?
The procedure is legitimate and necessary for genuine periodontal disease. The complaint is over-diagnosis. Measured pocket depths settle it — ask for them.
Can a hygienist clean my teeth without a dentist exam?
It depends on state law. Several states allow direct-access hygienists, which can be cheaper.
What if I haven't been in years?
Expect a longer first visit, a full set of X-rays and possibly more than one cleaning appointment. It is worth doing anyway, and no competent practice will lecture you about it.
