The annual physical is free, right up until it isn't. The difference between a preventive visit and a diagnostic one is worth a few hundred dollars a year, and it usually comes down to what gets said in the room.

What preventive covers at no cost

In-network preventive services required under the ACA carry no copay, no coinsurance and no deductible. That includes the wellness visit itself, immunizations, and a long list of screenings graded by the US Preventive Services Task Force.

The sentence that costs money: "While I'm here, my shoulder has been hurting." That gives the visit a diagnostic component, billed against your deductible. It is legitimate billing, not a trick — but you should know it before you say it. If the shoulder matters, book a separate visit. If it can wait, mention it next time.

Say this at the start

"I'd like today coded as my annual preventive visit. If something comes up that changes that, please tell me before we go there." Reasonable practices simply say yes, and it gives you standing to dispute a bill if the coding lands differently without warning.

What is not free

  • Blood panels beyond the screening set. A basic lipid and glucose screen is preventive; a broad wellness panel added on may not be.
  • Follow-up on an abnormal result. The screening test is preventive; the diagnostic work that follows is billed.
  • Anything out-of-network. The no-cost rule applies in-network only.
  • The specialist you get referred to. A new, billed encounter.

Getting value from fifteen minutes

Bring three things written down: your current medications including supplements, any symptom you have noticed even if it seems minor, and your top two questions. Ask for results to be released to the portal and for an explanation of anything flagged. Ask explicitly which screenings you are due for — that question most reliably surfaces something overdue.
